Go to www.carecredit.com
Click Healthcare Providers in the top right corner
Click the Provider Log-In button (if you have never logged in, call 800-859-9975 for log-in information)
Enter username and password
Click "Resource Center" from the global navigation bar
Once in the Resource Center, click "My CareCredit"
Choose "Order Supplies"
Enter the quantity needed in the field next to each item
Verify shipping address at the bottom of the page
Click SUBMIT
