Multi-Language Health History Forms from MetLife Dental
In the 90s, The University of the Pacific (UOP) Dental School developed health history forms in 13 languages for dentists to use when interviewing patients on their medical history. Seeing the need for this information across the country, MetLife partnered with UOP to continue development of these multi-language health history forms. They now offer forms in 40 languages.
The Multi-Language Health History project will improve future education and enhance the quality of oral health care, while at the same time acknowledging and respecting the diversity and cultural heritage of the patients. The project is supported by Dr. Peter Jacobsen, PhD, DDS, Director of Oral Medicine at the University of the Pacific, as well as the MetLife Dental Advisory Council and their review board consisting of practicing dentists, dental academicians, and corporate benefit administrators.
